HOW WE WORK WITH YOU
A Clinical Partner You Can Rely On
Working With GPs
We work as an extension of your care team — GP-aligned care plans, regular clinical updates, change-of-condition notifications and concise summary reporting back to the referring GP.
DVA Referral Support
We guide referrers through DVA Gold and White Card funded services and handle the administration, so eligible veterans receive care with no out-of-pocket cost.
Hospital Discharge Support
We commence care for complex discharges within 24–48 hours, coordinating directly with discharge planners to enable safe transitions and reduce avoidable readmissions.
Allied Health Collaboration
We coordinate with physiotherapy, occupational therapy, pharmacy and other allied health providers to deliver integrated, continuous care around the veteran.
Communication & Reporting
Clear, timely communication is core to how we work — structured reporting, documented assessments and proactive updates keep every member of the care team informed.
Clinical Escalation Pathways
Defined escalation pathways and early identification of concerns mean deterioration is recognised and acted on quickly, in coordination with the GP and treating teams.
